Care Initiatives Hospice is seeking a PRN Hospice Social Worker to provide patient-centric psychosocial care across Council Bluffs, Avoca, Dunlap, and nearby communities. This role emphasizes compassionate, home- and facility-based support, addressing emotional and spiritual needs with a holistic approach.

You will collaborate with families, caregivers, and the clinical team to assess needs, coordinate referrals, and promote programs that improve quality of life for patients in life-limiting

Qualifications

• Requires MSW from a CSWE-accredited program and active LMSW license.
• Hospice or palliative care experience preferred.
• Two years verifiable social work experience preferred.
• Valid driver's license and ability to travel between locations within the region.

Responsibilities

• Assess patients' psychosocial needs and complete social histories.
• Document care plans, progress, and follow-up per regulations.
• Provide counseling and emotional support to residents and families.
• Support patients and families with adjustment, rights, and orientation.
• Coordinate referrals and liaison with community resources.

• Plan and support patient, family, and community engagement activities.
• Promote facility programs and services to referral partners.

What it takes to be a Social Worker at Care Initiatives Hospice:

• Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education and current, valid social work license (LMSW).
• Hospice or palliative care experience, preferred.
• Two (2) years of verifiable experience working as a Social Worker, preferred.
• Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
