Job summary

Buckeye Hospice and Palliative Care Services is hiring a Medical Social Worker to deliver psychosocial support to patients and families under the Plan of Care. A sign-on bonus of $5,000 is offered. The role requires an Ohio social work license and relevant experience in healthcare or hospice settings.

Qualifications

• MSW or BSW with applicable supervision and licensure
• Must be licensed to practice social work in Ohio
• Experience in healthcare or hospice settings preferred

Responsibilities

• Provide Medical Social Services to patients/families per the Plan of Care
• Conduct psychosocial assessment within 5 days of admission
• Participate in IDG meetings and develop the patient’s plan of care
• Maintain and submit written clinical records (initial eval, care plan, daily notes)
• Assist with discharge planning and coordination with case managers
• Support patients and families in coping with dying and end-of-life care
