Job summary

UnitedHealth Group, through Heart 'n Home Hospice (LHC Group), is seeking a Social Worker to deliver medical social services under physician guidance within an Interdisciplinary Group. You will help patients and families understand social and emotional factors affecting health and support coping strategies, with a focus on care planning and resource linkage.

Primary responsibilities include assessing personal, emotional and environmental factors, contributing to a comprehensive Plan of Care, and

Qualifications

• Master's Degree from a CSWE-accredited School of Social Work.
• Current Driver's License, vehicle insurance, and access to dependable transportation.
• Licensed Social Worker in the state of Oregon (or eligible).
• 1+ years of social work experience in a health care setting.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Assists patients, family members, physicians and care teams to understand personal, emotional, environmental and social factors affecting health.
• Contributes to development of a comprehensive, integrated Plan of Care on a daily basis.
• Instructs care team on community resources available to assist patients.
