Qualifications

• Master's degree in Social Work from a CSWE-accredited program.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• OR: Licensed as a Social Worker in the State Board of Social Work.

Responsibilities

• Assists patient and care team to understand factors related to health status on an as-needed basis.
• Contributes to the development of a comprehensive Plan of Care for patients daily.
• Instructs team members on community resources to assist patients as needed.
