Job summary

Patriot Healthcare is seeking a Hospice Social Worker to support patients, families, and caregivers with emotional and practical needs. You will be part of the interdisciplinary team, providing needs assessment, counseling, and resource coordination to ensure quality end-of-life care.

The role requires an accredited MSW or BSW and an SC social work license (LBSW/LMSW/LISW-CP). PRN to full-time; Hospice experience preferred.

Qualifications

• Master’s or Bachelor’s degree in social work from a CSWE-accredited program.
• Licensed as LBSW, LMSW, or LISW-CP in the State of SC.
• Minimum of one year social service experience in a health care setting.
• Hospice experience preferred.

Responsibilities

• Provide social service interventions, supportive counseling, and discharge planning.
• Coordinate care with the interdisciplinary team, nurses, and caregivers.
• Assist patients and families with resources, networking, and crisis intervention.

Join our team as a Hospice Social Worker

Position Summary

The Hospice Social Worker role is responsible for managing the social service needs of our hospice patients, their families and caregivers. This includes all social service interventions and referrals, including needs assessment, supportive counseling, networking, resource procurement, crisis intervention and discharge planning. You will serve as a part of the interdisciplinary group, attending team conferences and coordinating care with the primary hospice nurses, family members and client services staff and volunteers.

Qualifications

Graduate of a Master’s or Bachelor’s program in social work accredited by the Council on Social Work Education; and is licensed as a LBSW, or LMSW, or LISW-CP by the State of SC. Must have, as a minimum, one (1) year social service experience in a health care setting with knowledge of death, dying, and bereavement. Hospice experience preferred.
