Job summary

Patriot Healthcare in South Carolina is seeking a Hospice Social Worker to manage the social service needs of our hospice patients and families, providing support, crisis intervention, and discharge planning as part of the interdisciplinary team.

Requirements include a CSWE-accredited MSW or BSW and state licensure (LBSW/LMSW/LISW-CP); at least one year in healthcare social work; hospice experience preferred.

Qualifications

• Master's or Bachelor's in social work from CSWE-accredited program.
• Licensed as LBSW, LMSW, or LISW-CP by SC.
• Minimum 1 year of social service experience in health care; hospice preferred.

Responsibilities

• Manage social service needs for hospice patients, families and caregivers.
• Provide supportive counseling, crisis intervention and discharge planning.
• Coordinate care with interdisciplinary team, nurses, and client services staff.
