Responsibilities

• Develop a bereavement plan of care for families.
• Educate patients and families and address barriers to learning.
• Maintain timely visit notes, orders, and EMR documentation.
• Collaborate with IDT to implement social work plans.
• Recognize end-of-life needs and Medicare Hospice benefits and eligibility.
