Job summary

Carle Health is seeking a Home Services Social Worker to assess the psychosocial needs of patients and families and to coordinate support through community resources and short-term counseling. The role involves collaboration with interdisciplinary teams and hours that vary with census and program needs.

The incumbent will provide bereavement planning, address barriers to care, and document assessments in the EMR while supporting culturally sensitive care and end-of-life decision making.

Qualifications

• Master’s Degree in Social Work required.
• Experience in hospice care.
• Licenses (LCSW or LSW) and appropriate state licensure may be required.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems.
• Develops a bereavement plan of care to address family member/caregiver needs.
• Documents psychosocial assessments and MSW interventions in the patient’s EMR.
• Interprets and communicates patient/family faith and culture traditions; supports self-determined life care decisions.
