Job summary

MJHS, a not-for-profit health system serving the Greater New York area, seeks a licensed social worker for hospice and palliative care support. You will provide psychosocial care to patients and families across inpatient, home care, and nursing home settings, including bereavement work, and collaborate with an IDG team to coordinate discharge planning.

The role requires an MSW with at least one year in healthcare social work, strong data entry skills, and familiarity with Word/Excel.

Qualifications

• Graduate from an accredited School of Social Work.
• Minimum of one year experience in Social Work in a healthcare setting.
• Knowledge of Word and Excel preferred.
• New York State MSW licensure.

Responsibilities

• You will provide psychosocial support and other Social Work services to Hospice patients and their families in the inpatient, home care/nursing home settings, in pre and post bereavement phases of Hospice service
• You will serve as social work member of hospice IDG and as liaison between hospice staff and inpatient and nursing home staff
• You will develop social work plan of care, goals and makes recommendations based on patient/family needs for assigned patients who are direct admission to inpatient units or nursing home/hospice residents.
• You will develop social work plan of care and discharge plans when appropriate for patients admitted into inpatient facility.
