Job summary

ExcelaCare is seeking a Masters of Social Work (MSW) to join our hospice team in Las Cruces, NM on a PRN basis. The role involves evaluating patient needs, coordinating social work services, and liaising with hospitals, Home Health, and community resources to ensure comprehensive patient care.

Qualifications

• 1 year social work experience in a health care setting.
• Current state issued social work license with current certification to practice in the state.
• Driver’s license and proof of auto liability insurance.

Responsibilities

• Evaluates and assesses patient needs for social service assistance.
• Plans and coordinates patient programs; liaises between patient, hospital, Home Health, Hospice, physician, and community.
• Responds to referrals from hospice care team.
• Interviews patients and/or caregivers to assess psychotherapy needs.
• Maintains and updates information on community resources.
• Completes documentation per Agency policies; follows Plan of Care for Medical Social Services.
• Attends visits and participates in case conferences.
