Job summary

AccentCare, Inc. in Colorado seeks a Hospice Social Worker (MSW I) to support home health patients in Loveland and surrounding areas, including Estes Park/ Lyons. This PRN role requires CO licensure (LSW/LCSW) and a Master’s in Social Work.

After-hours on-call and holiday coverage are included as part of a dedicated interdisciplinary team. You will assist with discharge planning, assess social and emotional needs, educate families on advance directives, and connect patients with community

Qualifications

• Master’s Degree from accredited school of Social Work or related field; CO licensure/permit to practice if required.
• Licensed to practice as a clinical social worker in CO.
• At least one year of professional social work experience in healthcare; 2+ years preferred.
• Medicare/Medicaid/Insurance specialty preferred.
• Experience or education in grief counseling preferred.
• Discharge planning experience preferred.

Responsibilities

• Assess social, spiritual, and emotional needs to estimate patient and caregiver capacity and to inform plan of care.
• Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
• Educate the patient and family on advance directives/advanced care planning.
• Utilize resources to support patient and family within illness limitations.
• Support the bereavement program.
• Discharge planning related to changes in level-of-care or placement.
