Job summary

Elara Caring is seeking a Master Social Worker (MSW) to support home-based care across our patient population. You will conduct social work evaluations, identify patient needs, and coordinate with the care team to implement safe, effective plans of care.

The role requires a Master’s or Doctoral degree in Social Work, 1+ year healthcare experience, and a valid state license. Expect up to 50% travel and collaboration with a Clinical Team Manager to drive quality outcomes.

Qualifications

• Excellent verbal and written communication skills.
• Social work skills as defined as general social work standards.

Responsibilities

• Performs initial social work evaluation visit and reevaluates needs at each visit.
• Identifies psychosocial, financial, and environmental needs of patients.
• Communicates findings to Supervisor and physician and other care personnel.
• Implements patient safety plans using community resources.
• Contributes to Plan of Care development for quality and continuity of care.
• Writes physician orders to adjust visits and care plans as needed.
