JourneyCare Hospice in Vernon Hills and Lake Forest, IL is seeking a compassionate Hospice Social Worker to guide patients and families through transitions within a busy care team.

The role requires a Social Work Degree with licensure and at least one year in hospice or medical social work; MSW strongly preferred. Excellent communication and coordination with community agencies are essential.

Responsabilidades

• Performs psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures needs are addressed in the plan of care.
• Provides exceptional service to patients, families, referral sources, co-workers and other customers.
• Counsels patient and family/caregivers and staff regarding stress and coping difficulties.
• Provides accurate and timely documentation of patient services reflecting the plan of care.
• Assists with applications for Supplemental Security Income, Hospice, Medicaid and Medicaid Waiver; connects with community resources.
• Serves as liaison between patients/families and community agencies.

About the Role

Qualifications:

• Social Work Degree
  • MSW strongly preferred
• Current State Licensure in Social Work
• Minimum of one year’s experience in hospice or medical social work in either hospital, palliative care or home health capacity is greatly desired.
• Valid driver’s license and proof of insurance is required

HOURS: Monday-Friday 8:30AM-5PM

On-Call Expectations: 3 shifts/month On-Call Rotation Afterhours and Weekends: Weeknights: 5PM-8AM and Weekends: 8AM-5PM and 5PM-8AM

What You’ll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies

Salary Range: $62,000-$80,000 - Offer based on experience
