Job summary

JourneyCare Hospice in Barrington, IL seeks a compassionate Hospice Social Worker to support patients and families through transitions and to guide care planning.

This role requires a Social Work Degree with current state licensure; MSW preferred. Mon-Fri 8:30AM-5PM with on-call shifts. Valid driver’s license and willingness to travel within the service area are required.

Qualifications

• MSW preferred and Social Work Degree required.
• Current State Licensure in Social Work is required.
• Minimum of one year’s hospice or medical social work experience is greatly desired.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Performs psychosocial assessments of patients and families/caregivers in the hospice program and ensures identified needs are addressed in the plan of care.
• Demonstrates exceptional service to patients, families, referral sources and coworkers.
• Counsels patient and family/caregivers and staff on stress and coping.
• Provides accurate and timely documentation of patient services to reflect the plan of care.
• Assists with applications for benefits and resources from community agencies to meet patient/family needs.
• Serves as liaison between patients/families and community agencies.

Qualifications:

HOURS

Monday-Friday 8:30AM-5PM

On-Call Expectations

1-3 shifts/month On-Call Rotation Afterhours and Weekends: Weeknights: 5PM-8AM and Weekends: 8AM-5PM and 5PM-8AM

What You’ll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies

Salary Range

$62,000-$80,000 - Offer based on experience
