Responsibilities

• Assesses psychosocial status and factors related to client’s illness and treatment.
• Provides direct social casework services including counseling and interventions.
• Advises agency personnel, clients, and families on resources and regulations.
• Participates in development and renewal of the client’s plan of care.
• Observes and reports client response to treatment to the physician.
• Maintains client care records and timely communication with care teams.
• Attends client care conferences, staff meetings, and committee meetings.
