Peace Bridge Hospice is seeking a dedicated Hospice Social Worker (MSW/LSW/LISW) to join our field-based team in Cincinnati. The role focuses on psychosocial support, care coordination, and end-of-life planning for patients and families in home hospice care.

Qualifications

• MSW required and active Ohio license (LSW or LISW)
• Experience in hospice, palliative care, home health, or healthcare setting preferred
• Strong understanding of end-of-life care and grief support
• Compassionate, patient-centered approach
• Ability to work independently in a home-based environment

Responsibilities

• Conduct comprehensive psychosocial assessments for hospice patients and families
• Provide emotional support, grief counseling, and crisis intervention
• Assist with end-of-life planning, advance directives, and goals-of-care discussions
• Educate families on hospice philosophy and available support services
• Connect patients to community resources, financial assistance, and social services
• Collaborate with nurses, physicians, chaplains, and hospice team members
• Facilitate communication between patient, family, and care team
• Provide bereavement and ongoing family support
• Maintain compliant documentation and participate in care team meetings
