Trinity Health At Home is seeking a Hospice Medical Social Worker (MSW) to provide compassionate, patient-centered support in patients' homes across Grand Rapids, MI. You will assess psychosocial needs, support care planning, and advocate for goals that promote dignity and independence.

Whether transitioning from hospital or currently in home health, you’ll deliver meaningful social work with autonomy, flexible scheduling and impactful 1:1 patient relationships, under clinical-led leadership.

Formación

• Master's Degree in Social Work from a CSWE-accredited program.
• Michigan MSW license required.
• Two years of healthcare social work experience.
• Ability to work with acutely and chronically ill patients.
• Ability to perform written, verbal, and observational tasks.

Minimum Qualifications

• Master's Degree from a school of social work accredited by the Council on Social Work Education.

• State of MichiganFull Masters Social Work License Required

• Two (2) years of social work experience in healthcare setting.

• Ability to provide service to acutely and chronically ill patients

• Ability to complete written, verbal, and observational tasks indigenous to the position.
