Job summary

Queen City Hospice in Fayetteville, OH is seeking a Medical Social Worker to provide psychosocial support to hospice patients and families, assess needs, coordinate resources, and help align services with the care plan.

This role emphasizes counseling for patients and staff, accurate documentation, and collaboration with an interdisciplinary team. A Bachelor's in Social Work is required; a Master's is preferred, with current Ohio licensure and at least one year of hospice experience.

Qualifications

• Bachelor’s degree in Social Work required; Master’s degree preferred.
• Alternative degrees: Psychology, Sociology, or related field.
• Current Ohio Social Work Licensure required (LSW, LISW, or LPSW-S).
• Minimum of 1 year of healthcare or hospice experience.
• Valid driver’s license and reliable transportation.
• Strong interpersonal, communication, and organizational skills.

• Conduct timely psychosocial assessments of patients and families/caregivers.
• Develop and support individualized care plans based on patient needs.
• Provide counseling to patients, families, and staff related to stress and coping challenges.
