Job summary

Optimal Care - Evergreen is seeking a Hospice Medical Social Worker (LMSW/LCSW required) in Jackson, MI. You will provide compassionate psychosocial care to patients and families facing end-of-life challenges, performing psychosocial assessments and counseling within a supportive interdisciplinary team.

The role requires licensure (LMSW or LCSW) and at least one year of experience, with daily travel to patients’ homes and facilities within the service area.

Qualifications

• Master's or doctoral degree from a CSWE-accredited school of social work.
• Current state licensure as an LMSW or LCSW.
• Minimum 1 year of experience as a licensed Master’s Social Worker.

Responsibilities

• Provide compassionate psychosocial care to help patients and families navigate end-of-life challenges.
• Conduct comprehensive psychosocial assessments, crisis intervention, and counseling.
• Support advance care planning conversations and collaborate with the interdisciplinary team.
