Optimal Care in Michigan is seeking a dedicated Hospice Medical Social Worker to support patients and families facing end-of-life decisions. You will perform psychosocial assessments, provide counseling, and collaborate with the interdisciplinary team to integrate emotional and practical needs into the plan of care.

This role requires LMSW or LCSW licensure, a CSWE‑accredited degree, travel to homes and facilities within the service area, and a compassionate approach to grief support.

Qualifications

• Master’s or doctoral degree from a CSWE‑accredited school of social work.
• Current LMSW or LCSW licensure.
• Minimum 1 year of experience as a licensed Master’s Social Worker.
• Willingness to travel to homes, facilities, and community settings.

Responsibilities

• Provide compassionate psychosocial care to patients and families navigating end‑of‑life illness.
• Complete comprehensive psychosocial assessments and crisis intervention.
• Support advance care planning conversations and guide values-based decisions.
• Collaborate with nurses, physicians, and hospice professionals to integrate emotional and practical needs into each patient’s plan of care.
