Job summary

Three Oaks Hospice seeks a Full Time Licensed Masters' Level Social Worker to provide comprehensive psychosocial assessments and counseling in home-based hospice care. The role partners with the IDG/IDT to coordinate patient care and support families through end-of-life planning.

The LMSW will maintain licensure, access community resources, and support patients across age groups while aligning with hospice philosophy and quality standards.

Qualifications

• Master’s degree in social work from an accredited program.
• Current LMSW license.
• Strong interpersonal and communication skills.
• Knowledge of community resources.
• Reliable transportation; valid driver’s license and current auto liability insurance

Responsibilities

• Assess and complete psychosocial assessments within five days of hospice election.
• Provide counseling to patients and families around end-of-life issues and resources.
• Coordinate care with IDG/IDT and other care team members.
• Promote hospice philosophy and patient-family advocacy.
• Maintain knowledge of eligibility standards and continuing education requirements.
