Job summary

Residential Home Health and Residential Hospice in Jackson, MI is seeking a licensed Hospice Medical Social Worker (MSW) to provide compassionate psychosocial care to patients in their homes. This role offers autonomy within an interdisciplinary team and meaningful, patient-centered outcomes.

You will conduct psychosocial and bereavement assessments, develop care plans, provide counseling, and help families navigate end-of-life decisions.

Qualifications

• MSW from an accredited graduate program.
• 1–2 years post-graduate social work experience in health care.
• Current license as a Medical Social Worker in the practice state.
• Valid driver’s license and ability to drive ~20% of day.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting with an interdisciplinary team.
• Conduct psychosocial and bereavement assessments and develop care plans.
• Provide counseling and guidance to patients and families, connecting them with resources.
• Apply critical thinking and ensure compliant EMR documentation.
• Work independently but never alone.
• Travel to patient homes within assigned territory.
