Qualifications

• Master of Social Work from an accredited program is required.
• Licensed as a Social Worker by the State Board of Social Work; current license required.
• Minimum one year experience in a healthcare setting; hospice experience preferred.

Responsibilities

• Assess and communicate psychosocial status to the RN Case Manager and the interdisciplinary team.
• Evaluate medical needs, home situations, financial resources, and community support.
• Implement personalized social work plans and collaborate with the Bereavement Coordinator.
• Shape care plans, attend interdisciplinary meetings, and address psychosocial stresses.
• Provide empathetic counseling to patients and families.
