Advocate Health's GCMG Hospice and Palliative team in Cabarrus, NC is seeking a full-time Social Worker. The role involves psychosocial assessments, brief psychotherapy, and care coordination within a multidisciplinary team. Master’s in Social Work and a Social Work license are required; 1–2 years healthcare experience is preferred.

Monday–Friday schedule, 40 hours/week. Base pay range is $30.70–$46.05 per hour, reflecting expertise and shift premiums as applicable.

Qualifications

• Master's Degree in Social Work from a CSWE approved graduate school.
• 1 year experience in the department or 2 years healthcare experience for external candidates.
• License in Clinical Social Work in applicable state and/or SC; certification preferred.

Responsabilités

• Provides biopsychosocial assessments and treatment planning.
• Interviews patient and social support to complete psychosocial evaluations.
• Provides brief psychotherapy and education to patients and families.
• Develops and coordinates care plans with multidisciplinary team.
• Advocates for patients and families throughout care continuum.
• Provides supervision to provisionally licensed social workers and mentors new staff.
