Job summary

Crescent Hospice is seeking a dedicated Licensed Master Social Worker (LMSW) to deliver patient-centered social work services to hospice patients and families. Qualifications

• Master of Social Work from an accredited school of social work.
• Licensed as a Social Worker with the State Board of Social Work.
• Minimum one year experience in healthcare; hospice experience preferred.

Responsibilities

• Be a supportive guide: assess psychosocial status and communicate with the RN Case Manager and team.
• Evaluate patient home situations, finances, and community supports.
• Develop individualized social work plans and coordinate with Bereavement Coordinator.
• Participate in interdisciplinary meetings and support the care plan.
• Provide empathetic counseling to patients and families.

Responsibilities

Qualifications

Education

• Required: Master of Social Work from an accredited school of social work

License Requirements

• Required: Licensed as a Social Worker from the State Board of Social Work
• Required: Current Driver's License, vehicle insurance, and access to a dependable vehicle

Experience

• Preferred: Minimum one (1) year experience as a social worker in a hospice setting
• Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred

$31.00 - $33.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate's relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
