Job summary

Catholic Palliative Care Services seeks a Social Worker to provide psychosocial support in our Inpatient Unit, with visits on a visiting and intermittent basis. You will assess needs, counsel patients and families, and develop care plans in collaboration with the interdisciplinary team.

The role requires an MSW, active Florida license, and 1–2 years of health care social work experience (hospice preferred).

Qualifications

• Master’s Degree from a social work program.
• Active license to practice in Florida.
• 1–2 years social work experience in health care; hospice preferred.

Responsibilities

• Assess patient needs in the Inpatient Unit within regulatory requirements.
• Provide individual counseling to patients and family members coping with terminal illness.
• Develop treatment plans with the interdisciplinary team.
• Complete documentation and bereavement assessments.
• Coordinate with community and hospital resources for patient needs.
