Qualifications

• LMSW or BSW is required; LCSW preferred.
• Master's or Bachelor's Degree in social work, social sciences, or related field.
• One year of clinical experience with emphasis on case work, family processes and group work.

Responsibilities

• Provide psychosocial care to patients and families as part of the hospice interdisciplinary team.
• Support emotional, spiritual needs of patients and caregivers.
• Collaborate with team to plan and implement individualized care plans.
