Job summary

VIA Health Partners, a community-based hospice and palliative care provider in Charlotte, NC, seeks a licensed social worker to deliver comprehensive psychosocial assessments, develop and implement care plans, and collaborate with interdisciplinary teams to support patients and families facing end-of-life care. The role requires MSW from a CSWE-accredited program with LCSW/LCSWA licensure in NC/SC (or eligibility).

Hospice experience is preferred.

Qualifications

• MSW from a CSWE-accredited program
• Current LCSWA/LCSW licensure in NC/SC or eligible
• Computer proficiency and EMR/documentation skills
• Hospice or palliative care experience preferred

Responsibilities

• Conduct psychosocial assessments of patients, caregivers, and families.
• Develop, implement, and evaluate plans of care with an emphasis on counseling and education.
• Collaborate with interdisciplinary teams to coordinate patient-centered care.
• Document assessments and plans per VIA guidelines and ensure timely records.
• Assist with transitions to grief services and community referrals.
