Job summary

Domnovate Healthcare is seeking a licensed social worker to provide home-based hospice care, helping patients and families navigate options, connect with support, and cope with difficult decisions. You will work with a multidisciplinary team across community care settings. The assignment is in Albany, Oregon, a 13-week W-2 contract with weekday days from 8 a.m.

to 4:30 p.m. No weekends. 24 hours per week are guaranteed within a 40-hour planning week, and travel between patient locations is

Qualifications

• Active Oregon LMSW or CSWA license at submission.
• LCSW preferred.
• Healthcare-provider BLS and at least one year of relevant experience.
• Valid driver license, insured personal vehicle and ability to travel between patient care locations.
• Hospice experience preferred.

Responsibilities

• Provide home-based hospice care and support to patients and families.
• Collaborate with a multidisciplinary team across community care settings.
• Travel between patient care locations as required; participate in holiday coverage if needed.
