Job summary

Three Oaks Hospice is seeking a Licensed Master’s Level Social Worker to provide in-home psychosocial support across South Atlanta areas, including Jonesboro and Fayetteville. The role emphasizes assessments, counseling, and coordination with the IDG/IDT to plan patient care.

You will ensure adherence to hospice policies, assist with directives and discharge planning, and support families through end-of-life transitions within a collaborative team environment.

Qualifications

• Master’s degree in social work from a State accredited program.
• Current and valid LMSW license.
• Working knowledge of community resources. Good interpersonal skills.
• Current State Certification.
• Reliable transportation. Valid Driver’s License and current auto liability insurance.

Responsibilities

• Conduct psychosocial assessments and referrals to meet patient/family needs.
• Assist with advance directives, funeral planning, discharge planning, and family counseling.
• Maintain effective verbal and written interpersonal communication.
• Participate in IDG/IDT meetings and patient care planning.
• Support compliance with Hospice QAPI and infection control policies.
