Emblem Hospice in the Phoenix area is seeking a Hospice Social Worker for Scottsdale/Fountain Hills. The role supports patients and families with psychosocial assessments, counseling, and resource navigation across a Monday–Friday schedule in the Scottsdale and Fountain Hills areas.

You will collaborate with an interdisciplinary team to ensure holistic and dignified end-of-life care, while pursuing ongoing education and certifications. A CSWE-accredited MSW and AZ licensure are required.

Formación

• Master's degree in Social Work from a CSWE-accredited program.
• Active license to practice social work in Arizona.

Responsabilidades

• Comprehensive assessments evaluating psychosocial status of patients and families related to terminal illness and living environment.
• Care planning: develop and implement interventions based on thorough evaluations.
• Counseling & Support: guide patients and caregivers through stress, coping challenges, and end-of-life issues.
• Documentation: maintain accurate, timely clinical records for all referred patients.
• Resource navigation: provide referrals and liaise with community agencies.
• Team collaboration: participate in interdisciplinary care planning and regular team meetings.
• Advocacy: help physicians and team understand social and emotional factors impacting care.
• Quality improvement: contribute to performance improvement initiatives.
