Job summary

Qualifications

• MSW degree required.
• Wyoming LSW or LCSW license active and unrestricted.
• Strong understanding of social work theories and ethics.

Responsibilities

• Provide emotional, psychological, and social support to patients and families and develop individualized care plans.
• Collaborate with physicians, nurses, chaplains, and other professionals for comprehensive care.
• Address social determinants of health such as housing, transportation, and finances.
• Facilitate communication for informed patient and family decisions.
• Educate families on end-of-life care, coping, and bereavement.
• Document interactions and assessments per agency policies.
