Agape Care Group in North Charleston, SC is seeking a hospice medical social worker dedicated to compassionate end-of-life support for patients and families. You will perform psychosocial evaluations, provide counseling, and collaborate with the plan of care to ease emotional stress at difficult times.

The role offers support from a leadership team and opportunities for professional development with potential for advancement within our care network.

Qualifications

• MS degree in social work from an accredited school approved by the Council on Social Work Education (CSWE)
• Current state license as a social worker
• 2+ years of clinical work experience, preferably in healthcare or hospice

Responsabilités

• Conduct psychosocial evaluations and provide ongoing counseling for patients and families during end-of-life care.
• Support patients and families emotionally in alignment with the plan of care.
• Collaborate with care teams to ensure compassionate, holistic support.
