Heart’n Soul Hospice in Charleston, SC is seeking a dedicated Social Worker to assess patient and family psychosocial needs and provide counseling, crisis intervention, and planning support.

You will collaborate with the care team, coordinate community resources, document promptly, and ensure compliance with laws and policies. A state social work license and a degree are required, with hospice experience preferred.

Qualifications

• Bachelor's or Master's degree in social work from a State accredited program.
• Current State license required in accordance with State regulations.
• Current Driver's License, reliable transportation, and provide ongoing valid and current auto liability insurance.
• One year of experience as a Social Worker in a clinical care setting required, hospice experience preferred.

Responsabilités

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by POC documentation, clinical records, team meeting minutes and community resource referrals.
• Provide individual patient/family counseling, crisis intervention, information and assistance with preparation of advance directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments that are consistent with education and competence to meet the needs of the patients.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Timely submission of all documentation.
• Performance of other duties as required.
• Ensure overall compliance with local, state and federal laws, Medicare regulations, and established personnel policies and procedures.
