Your Health Organization in Spartanburg, SC is seeking a Licensed Social Worker to support hospice patients in homes and facilities. This role requires LMSW/LBSW licensure and involves daily travel; it is a full-time position with potential monthly bonuses and standard weekday hours.

Responsibilities include counseling, care planning, and collaboration with the care team to address medical and hospice needs, as well as facilitating access to social services and community resources.

Qualifications

• MSW licensure LMSW or LBSW in good standing.
• Hospice/healthcare experience preferred (1 year).
• SUD certification or experience preferred.

Responsabilités

• Document updates of ongoing care plans.
• Screen for mental health conditions and social determinants of health.
• Provide individual, couples and family counseling for mental health needs.
• Coordinate care plans with the care team and ensure integration.
• Educate patients and families about mental health options and resources.
