Job summary

Bristol Hospice is seeking a dedicated Hospice Social Worker to support patients and families at the end of life in Corvallis, OR. The role emphasizes compassionate psychosocial assessments, planning, and liaison with the interdisciplinary team to ensure comfort and dignity.

The position requires an MSW, at least two years of health care experience, understanding of hospice care, and willingness to travel within the service area; certification in hospice social work is a plus.

Qualifications

• Must have a master’s degree in social work from an accredited university (MSW preferred).
• Minimum of two years of supervised experience in healthcare; hospice experience preferred.
• Understanding hospice philosophy and death/dying issues; CHPN certification desirable.
• Willingness to travel throughout Bristol Hospice service area; flexible in work hours.

Responsibilities

• Assess the psychosocial status of patients and families and communicate findings to the nursing team.
• Conduct social evaluations including family dynamics, caregiver abilities, and risk factors; plan interventions.
• Counsel patients and families on stress and coping; provide crisis intervention when needed.
• Educate teams on patients’ cultural needs and preferences; educate on advance directives.
