Job summary

Bristol Hospice is seeking a dedicated Hospice Social Worker to support patients and families at a challenging time, guiding decisions with compassion and respect. This role emphasizes psychosocial assessment, care coordination, and collaboration with an interdisciplinary team to ensure comfort and dignity.

Candidates must have a Master’s in Social Work and supervised health-care experience; certification is desirable.

Qualifications

• Master's degree in social work (MSW) from an accredited university.
• Minimum of two years of documented supervised experience in health care; hospice experience preferred.
• Certified Hospice and Palliative Social Worker desirable.
• Flexible in work hours and able to travel throughout the Bristol Hospice service area.
• Willing to maintain knowledge of information systems and applicable software.

Responsibilities

• Assess the psychosocial status of patients and families and communicate findings to the nursing and interdisciplinary team.
• Perform social evaluations including family dynamics and caregiver abilities to plan interventions.
• Counsel patients and families, provide crisis intervention when necessary.
• Educate interdisciplinary group on cultural needs and family dynamics related to health and end-of-life issues.
• Maintain clinical records and assist in planning for advanced directives and funeral/financial decisions.
• Participate in quality improvement activities and development of individualized care plans.
