Job summary

Andwell Health Partners seeks a Hospice Registered Nurse (RN) for per diem home-based care in Farmington, ME and surrounding towns. You will provide skilled nursing care at patients' homes, guided by physician plans, to comfort and support families.

Requirements include Maine RN license and at least one year of clinical experience. You should be able to travel locally and collaborate with an interdisciplinary hospice team to deliver compassionate, patient-centered care.

Qualifications

• Current Maine RN license and valid clinical practice allowed.
• Minimum of one year of recent clinical nursing experience.
• Ability to travel locally to Farmington and surrounding towns.

Responsibilities

• Deliver skilled nursing in patients' homes per physician plans of care.
• Assess physical, emotional, psychosocial, and spiritual needs.
• Develop individualized care plans with measurable outcomes.
• Manage pain and symptoms to maximize comfort and quality of life.
• Educate patients and families to support informed decisions.
