Knowledge of pain management and symptom control practices.

Responsibilities

Deliver skilled nursing services in patients' homes per physician-directed plans of care.
Assess physical, emotional, psychosocial, and spiritual needs of patients.
Develop individualized care plans with measurable outcomes with patients and families.
Manage pain and symptoms to maximize comfort and quality of life.
Educate patients and families to support informed decisions.
Advocate for patient rights and goals of care.
Collaborate with interdisciplinary hospice team to provide holistic care.
Offer guidance, reassurance and compassionate support during challenging times.
