Transitions Hospice is seeking a Licensed Social Worker to provide compassionate psychosocial support to patients and families across Columbus and surrounding areas. You will conduct assessments, coordinate resources, and guide families through advanced directives.

Join an interdisciplinary team that values compassionate care, thorough documentation, and strong communication. This role requires a Master’s in Social Work and state licensure, with local travel required.

Competenze

• Master’s degree in social work from a CSWE-accredited program.
• Current state licensure as required (LSW/SSW/LCSW).
• Minimum 2+ years in health care or social services.
• Experience in hospice or palliative care preferred.
• Strong communication and grief support skills.
• Ability to travel locally for patient visits.

Mansioni

• Conduct initial psychosocial, spiritual, and pre-bereavement assessments.
• Provide ongoing emotional support and counseling.
• Connect families with community resources and financial assistance.
• Guide families through Advanced Directives with clarity.
• Assist with facility placement when appropriate.
• Maintain accurate documentation in patient records.
• Participate in interdisciplinary team meetings.
• Assist with evening and weekend on-call duties as needed.
• Ensure compliance with laws and Medicare regulations.
