Job summary

Caris Health is seeking a Social Worker to develop and deliver social work services that promote access to funding for patients in need and support emotional and social well‑being of patients and families. The role emphasizes collaboration with the interdisciplinary team to formulate care plans at admission and throughout care.

Qualifications

• Virginia: MSW preferred, BSW required and two years experience.
• One year of social work experience in a health care setting.

Responsibilities

• Conducts psychosocial assessments of patient and family and initial bereavement assessment within five days of admission.
• Discusses POC with admitting RN on day of admission.
• Performs regular social work visits; documents assessments, interventions, and outcomes.
• Assists with insurance needs; evaluates coverage and helps obtain Medicaid or other payer sources.
• Coordinates with Volunteer Coordinator for volunteer services; participates in IDT meetings.
