Job summary

Heart to Heart Hospice seeks a dedicated Social Worker to support patients and their families, coordinating psychosocial care and planning. You will assess needs, provide counseling, and help with advance directives and funeral planning issues.

This role requires a Bachelors or Master’s in social work, a current state license, a valid driver’s license, and at least one year of clinical social work experience; hospice experience is preferred.

Qualifications

• Bachelor's or Master's degree in social work from a State accredited program.
• Current State license required in accordance with State regulations.
• Current Driver's License and reliable transportation with current auto liability insurance.
• One year of experience as a Social Worker in a clinical care setting; hospice experience preferred.

Responsibilities

• Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs as evidenced by documentation.
• Provide individual patient/family counseling and crisis intervention, and assist with advance directives and funeral planning issues.
• Facilitate community resources based on patient/family needs.
• Accept clinical assignments consistent with education and competence.
• Perform on-call responsibilities and provide on-call service to patients/families.
• Timely submission of all documentation.
• Perform other duties as required.
• Ensure compliance with local, state and federal laws, Medicare regulations, and personnel policies and procedures.
