Job summary

Heart'n Soul Hospice is seeking a licensed social worker to join our team in providing compassionate, patient- and family-centered care. You will assess psychosocial needs, provide counseling, and coordinate resources to support individuals facing life-limiting illness.

Requirements include a Bachelor’s or Master’s in Social Work from an accredited program, current state license, and a valid driver’s license with reliable transportation.

Qualifications

Bachelor's or Master's degree in Social Work from a State accredited program.
Current State license required in accordance with State regulations.
Current Driver's License with reliable transportation.

Responsibilities

Assess hospice patient to identify psychosocial, grief, financial, environmental and community needs.
Provide individual patient/family counseling and crisis intervention.
Assist with advance directives, funeral planning, and related legal/fiscal issues.
Facilitate community resources based on patient/family needs.
Accept clinical assignments within education/competence.
Perform on-call responsibilities as assigned and timely documentation.
Ensure compliance with laws, Medicare regulations, and policies.
