PH Healthcare seeks a Hospice Social Worker to support patients, families, and caregivers with psychosocial assessments, counseling, and referrals within our hospice program.

The role involves interdisciplinary collaboration, crisis intervention, and discharge planning to ensure compassionate, patient-centered care. Experience in healthcare and knowledge of death, dying, and bereavement are valued.

Qualifications

• Must have a MSW or BSW from a CSWE-accredited program.
• Must be licensed as LBSW, LMSW, or LISW-CP in South Carolina.
• Minimum of 1 year social service experience in healthcare; hospice experience preferred.

Responsabilités

• Stewards the PH Healthcare mission, focusing on dignity and respectful response to life-threatening/terminal illnesses.
• Evaluate referrals and hospice admissions as directed.
• Assess psychosocial needs of patients and families within 72 hours of admission; interview patients, families, and staff.
• Participate with the interdisciplinary team to develop and update the plan of care biweekly.
• Coordinate psychosocial care to meet family needs in a meaningful way.
• Visit or contact patients and families as outlined in the plan of care.
• Maintain required documentation and ensure compliance with policies.
• Arrange access to community resources to resolve psychosocial issues.
• Assist with spiritual, bereavement, and volunteer needs for patients and families.
• Help team understand social and emotional factors affecting coping and care plans.
• Conduct individual, family, or group counseling as indicated.
• Provide resources and education to families and colleagues; involve family in planning.
