Angels Care Hospice seeks a qualified social worker to implement standards of care for medical social work services in a hospice setting. The role focuses on assessing psychosocial status, coordinating assessments, counseling families, and liaising with community agencies to support patient care.

Qualifications

• MSW degree or equivalent in social work.
• Licensed or eligible for licensure as an LCSW.
• Experience in medical social work or hospice settings is preferred.

Responsabilités

• Assess psychosocial status of patients and families/caregivers related to terminal illness.
• Evaluate residence safety and care needs; plan interventions as required.
• Conduct social evaluations including family dynamics and caregiver abilities.
• Counsel patients, families and caregivers on stress and coping strategies.
• Provide crisis intervention when necessary.
• Educate interdisciplinary teams on cultural considerations for patients and families.
• Maintain clinical records and progress notes for referred patients.
