Qualifications

Responsibilities

• Assesses the psychosocial status of patients and communicates findings to the registered nurse and other members of the interdisciplinary group.
• Carries out social evaluations and plans intervention based on evaluation findings; provides crisis intervention when necessary.
• Educates on cultural needs related to patient and family; includes communication, family role, space, and taboos.
• Maintains clinical records on all patients referred to social work.
• Discharge planning when needed; participates in interdisciplinary meetings.
• Provides information and referral services to patients, families, and community agencies.
• Serves as liaison between patients, families, and community resources.
