Job summary

LoveWell Hospice seeks a Social Worker to provide coordinated home-based hospice care. You will perform psychosocial assessments, offer counseling and referrals, and collaborate with a broader team to implement individualized care plans.

You will participate in team meetings, stay updated on community resources, and support patients and families in navigating emotional, social, financial, and environmental needs within a compassionate, patient-centered approach.

Qualifications

• MSW from an accredited program approved by the state.
• Current Texas Social Work licensure/certification in good standing.
• Valid Texas Driver's License.

Responsibilities

• Conduct comprehensive psychosocial assessments to identify patients' and families' emotional, social, financial, environmental, and community resource needs.
• Provide counseling, support, advocacy, and resource referrals based on assessed needs and the plan of care.
• Collaborate with the interdisciplinary team to develop, implement, and evaluate individualized patient care plans.
• Attend and actively participate in interdisciplinary team meetings and patient care conferences.
• Promote and maintain knowledge of public and private assistance programs and community resources.
