Job summary

Carle Health is seeking a Home Services Social Worker to identify psychosocial needs of patients and families, coordinate with interdisciplinary teams, and provide supportive counseling in a hospice care context. Responsibilities include assessments, bereavement planning, and helping families navigate resources and benefits.

Qualified candidates will have a Master's in Social Work, LSW and/or LCSW licenses in Illinois, and a valid driver's license. Hours vary with census and program needs.

Qualifications

• Master's Degree in Social Work is required.
• LCSW, LSW, and IDFPR license/certification are required or preferred.
• Driver's License and automobile insurance may be required.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop a bereavement plan of care addressing family/caregiver needs.
• Educate patients and families to overcome barriers and align care with beliefs and values.
• Document assessments and interventions in the EMR and coordinate with the IDT.
• Support end-of-life planning and access to community resources.
