Job summary

Bristol Hospice in Madison, WI is hiring a Hospice Social Worker. This full-time role conducts psychosocial assessments, provides family counseling, and coordinates with the care team to deliver compassionate end-of-life support.

Qualifications

• Must have a master’s degree in social work from an accredited university.
• Certified Hospice and Palliative Social Worker is desirable.
• Must be flexible in work hours and able to travel throughout Bristol Hospice service areas.

Responsibilities

• Assess the psychosocial status of patients and families/caregivers related to the terminal illness and communicate findings.
• Counsel patients and families, provide crisis intervention as needed.
• Educate and collaborate with the interdisciplinary team on cultural needs, advance directives, and care planning.
• Maintain clinical records for all patients referred to social work and participate in quality improvement activities.
