Job summary

Bristol Hospice - Madison LLC in Wisconsin seeks a compassionate Hospice Social Worker to assess psychosocial needs and support families during end-of-life care. You will collaborate with the interdisciplinary team to ensure compassionate, dignified care aligned with patient goals.

The role requires a Master’s degree in social work and at least two years of supervised healthcare experience (hospice experience preferred).

Qualifications

• Must have a master’s degree in social work from an accredited university.
• Two years of supervised experience in health care, hospice experience preferred.
• Certified Hospice and Palliative Social Worker is desirable.
• Flexible in work hours and ability to travel within Bristol Hospice service area.

Responsibilities

• Assess psychosocial status of patients and families and communicate findings to the interdisciplinary team.
• Provide assessments in patient residences and plan safety when the home is not safe.
• Conduct social evaluations including family dynamics and caregiver abilities; plan interventions.
• Counsel patients and families; provide crisis intervention.
• Educate the team on cultural needs and communication; maintain records for referrals.
• Assist with advanced directives information and patient/family education.
• Provide information and referrals to patients, families, and community agencies.
• Serve as liaison between patients, families, and community resources.
• Participate in care planning with patient and family and attend interdisciplinary group meetings.
• Support funeral, financial, and healthcare decision planning.
